Learning from Failures Johann F. Szautner, PE, PLS/Civil & Forensic Engineering Consultant
The objective of this session is to integrate a risk management based approach into engineering design and failure avoidance, with the goal to eliminate or reduce the risk of injury and/or economic loss. You will learn failure avoidance by learning to identify and analyze hazards and risks, framework for reliable design, and safety through design.

Failure Investigation for Engineers JamAaron Tanzer, PE/Lehigh Testing Labs This course will focus on the process involved in performing deductive logic-based investigations. Differentiating the likely goals of such investigations (i.e. failure mode, failure mechanism, root cause) and how they influence investigation organization will be discussed. Topics covered will include sample receipt and handling, general process overview from least to most destructive testes, organization of test protocols (including common tests), and running changes to testing based on test results. Special issues concerning litigation-related analyses will be discussed. Examples will be used to illustrate these concepts. Questions will be raised during the course for group discussion.
